Adopting a friend.............CASI has many healthy, lovable dogs/cats available for adoption.  All have received necessary medical attention, including vaccinations, worming, heartworm or leukemia testing and spaying or neutering.

ADOPTION APPLICATION (MS Word format)

CASI is dedicated to the humane and loving treatment of stray and homeless animals. CASI is an all volunteer organization that operates entirely on generous donations from caring individuals and friends.  Therefore, it is the responsibility of the adoptive family to provide food, housing and medical care, as needed, for the animal they have adopted.  We request that you do not ask us to pay for medical care or any other expenses you may incur by being a pet owner.  The animal becomes your responsibility when you leave our shelter. Please be sure you can provide the necessary care before you adopt.  We are confident that all of the animals we adopt are healthy and in good physical condition (unless otherwise stated).
  

	Our dogs and cats have been fully vaccinated and been given worming medication.
	Dogs and cats over the age of 6 months have been spayed/neutered.
	When adopting puppies/kittens you are required to sign a sterilization agreement.
	Dogs over the age of 6 months have been heartworm tested.
	Cats over the age of 6 months have been tested for feline leukemia.
	Our cats are litter boxed trained and very social. We recommend they be adopted as inside cats. This is the only way you can insure their safety.
